In consideration of being allowed to participate in any way in the Aledo Soccer Club Summer Camp (“Camp”) on May 26–28, 2025, the undersigned acknowledges, appreciates, and agrees that:

I understand that participation in soccer and related activities involves inherent risks, including but not limited to, the risk of injury, illness, or property damage.
I, for myself and on behalf of my child(ren), hereby voluntarily assume all such risks, both known and unknown, even if arising from the negligence of Aledo Soccer Club, its directors, coaches, volunteers, agents, or other participants.
I agree to release, indemnify, and hold harmless Aledo Soccer Club, Frontrunner Fieldhouse, affiliates, sponsors, and respective officers, agents, and employees from any and all claims, liabilities, damages, or causes of action arising out of or connected with participation in this Camp.
I acknowledge that Aledo Soccer Club does not carry or provide medical or accident insurance for participants and that I am responsible for any medical expenses incurred.
I consent to the use of photographs or video recordings of my child(ren) for promotional purposes without compensation.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND SIGN IT FREELY AND VOLUNTARILY.

Medical Consent(Required)

I acknowledge and accept the terms of the liability waiver.

Medical Authorization & Emergency Treatment Consent

I, the undersigned parent or legal guardian of the participant named below, hereby authorize Aledo Soccer Club staff, coaches, volunteers, or representatives to act on my behalf in seeking emergency medical treatment for my child in the event I cannot be reached.

I authorize any licensed physician, emergency medical technician, hospital, or medical facility to provide any necessary medical care.
I understand that every effort will be made to contact me prior to treatment, but in case I am unavailable, I grant permission for medical treatment as deemed necessary.
I agree to assume full financial responsibility for any medical services rendered.

Photo/Video Release(Required)

I consent to photos/videos of my child being used for promotional purposes.
