WAIVER AND RELEASE OF LIABILITY

In consideration of being allowed to participate in the Aledo Soccer Club Open Training Sessions, I, the undersigned parent/guardian, acknowledge, appreciate, and agree to the following:

I understand that soccer activities involve inherent risks including but not limited to: physical contact with other participants, falls, collisions, and injuries from soccer balls and equipment. I voluntarily assume all risks associated with participation.

I hereby release, waive, discharge, and covenant not to sue Aledo Soccer Club, Frontrunner Fieldhouse, Aledo Middle School, Aledo ISD, their officers, directors, coaches, volunteers, and employees from any and all liability, claims, demands, or causes of action that may arise from my child's participation.

I confirm that my child is physically fit to participate in soccer activities. I grant permission for emergency medical treatment if necessary. I understand that I am responsible for my child's conduct during sessions.

I consent to the use of photographs and video recordings of my child taken during sessions for promotional purposes by Aledo Soccer Club and Frontrunner Fieldhouse.

By checking the box below, I acknowledge that I have read this waiver, understand its contents, and agree to its terms on behalf of myself and my minor child.
